The calcineurin inhibitors (CNIs), tacrolimus and ciclosporin, both metabolised by the enzyme CYP3A4 and P-glycoprotein, will be the first-line immunosuppressant medications used to avoid organ rejection 15. Drug–drug interactions can lead to changes that are significant bloodstream plasma amounts and mainly occur when medications which are either inducers or inhibitors of this enzyme cytochrome P450 3A4 (CYP3A4) are prescribed (see Table 2).
For patients recommended ciclosporin and tacrolimus, pharmacists should search for prospective drug–drug interactions along with medicines that the in-patient is recommended.
